Most vision plans cover the cost of one exam per calendar year and provide additional coverage to help offset the cost of lenses and frames or contacts. Indemnity Plans -  You may visit any provider you choose, but you will be required to cover the cost of care in full at the time of service.

PPO - When you visit an in-network provider, you will save more. In-network providers have agreed to negotiated rates, meaning you will pay less when you seek care.

HMO - You are typically required to designate an in-network primary care provider and receive care only from them for vision services. Out-of-network coverage is not available.
